About the CompanyThe Senior Medical Social Worker (Senior MSW) is a senior member of the Social Services Department responsible for providing therapeutic psychosocial interventions, complex case management, programme development, and team supervision within the nursing home setting. The Senior MSW provides leadership in the management of high-risk and complex cases, supports interdisciplinary care planning, and contributes to the development of resident and caregiver support initiatives aligned with person-centred care principles and organisational goals. The Senior MSW also supports service quality, staff management and capability development, and operational coordination within assigned clusters or services.
About the RoleThe Senior Medical Social Worker (Senior MSW) is responsible for providing therapeutic psychosocial interventions, complex case management, programme development, and team supervision within the nursing home setting.
Responsibilities
SPECIFICS (80%) Advanced Case Management & Psychosocial Intervention Lead psychosocial assessment and intervention for residents and families with complex, high-risk, or high-profile needs (e.g., elder abuse, APS referrals, suicidality, severe family conflict, behavioral concerns, mental health issues, cross-agencies case management).Involved in casework and apply systemic Biopsychosocial-Spiritual (BPSS) frameworks in case formulation and intervention planning.Develop and implement psychosocial care plans in collaboration with interdisciplinary teams.Conduct family conferences, mediation, grief support, supportive counselling, and crisis intervention where appropriate.Prepare comprehensive social reports and ensure documentation quality and clinical standards are maintained.Support care transition planning and coordination with hospitals, community agencies, and relevant stakeholders to ensure continuity of care.Provide consultation and guidance to junior staff on case formulation and intervention approaches.
Programme Design, Development & Execution Plan, develop, coordinate, and evaluate psychosocial and therapeutic programmes that support residents’ psychosocial well-being, meaningful engagement, and quality of life.Design and implement caregiver support initiatives, caregiver education programmes, and community engagement activities.Work collaboratively with interdisciplinary teams and external partners to develop programmes responsive to residents’ and caregivers’ evolving needs.Monitor programme outcomes and recommend improvements based on service trends, resident feedback, and organizational priorities.Support organisation-wide person-centered care and care transformation initiatives through programme innovation and service development.
Team Leadership & Supervision Provide guidance, mentoring, and supervision to Medical Social Workers, Social Work Associates (SWAs), administrative staff, and/or students where assigned.Support Team Leads in overseeing workflow coordination, case allocation, and service delivery standards across assigned clusters or centres.Monitor team performance and support capability development through coaching, case discussions, and reflective practice.Escalate high-risk or complex issues appropriately and support implementation of follow-up actions.Foster a collaborative, professional, and learning-oriented team culture.
Governance, Quality & Service Development Ensure compliance with departmental SOPs, professional ethics, regulatory standards, and documentation requirements.Support departmental audits, quality improvement initiatives, and policy implementation.Participate in service planning, workflow improvement, and departmental projects.Contribute to the development and review of psychosocial care frameworks, SOPs, and operational guidelines.Support data tracking, reporting, and analysis related to psychosocial services and programme outcomes.
Collaboration & Community Partnerships Collaborate with healthcare professionals, hospitals, community care agencies, and external partners to support integrated and coordinated care.Participate in interdisciplinary meetings, case conferences, and stakeholder discussions.Support upstream caregiver engagement and community outreach initiatives to strengthen care continuity and caregiver support.
GENERAL (20%) Any other related duties as assigned by the reporting officers.
QualificationsBachelor’s Degree in Social Work (BSW).Registered or eligible for registration with the Singapore Association of Social Workers (SASW).
Required SkillsStrong competency in psychosocial assessment, case formulation, and clinical documentation.Experience in programme planning, implementation, and evaluation.Knowledge of healthcare, community care, subsidy, and safeguarding frameworks.Competent in IT and documentation systems.
